A “Home Plus” is a senior care home limited to no more than 12 residents at a time with access to 24/7 care in a safe environment designed for seniors.  At Fern’s Place Home Plus, we are committed to empowering seniors and their families to take control of their lives by offering a place for optimal living, preventing loneliness, providing access to heathy meals, and improving the well-being of everyone who takes advantage of the unique space known as “Fern’s Place.”

Fern’s Place is characterized by custom care plans that fit your needs.  A resident at Fern’s Place may require simple services like medication management, meals, housekeeping, and laundry services or may require more complex services including bathing, dressing, ambulation, and transfer.  In some cases, residents may also require short term skilled services such as acute pain management, physical, occupational, or speech therapy. These services are contracted through trusted outside service providers and healthcare partners.  Regardless of a resident’s specific needs, each resident’s custom care plan will ensure you or your loved one receives the care they need and deserve.
